Chronic-disease management

We specialize in helping seniors stay well with conditions like type 2 diabetes, high blood pressure, high cholesterol, heart disease, atrial fibrillation, COPD, osteoporosis, arthritis, and thyroid disorders. We coordinate labs, adjust medications, and refer to specialists when needed — and we follow up so nothing falls through the cracks.

Geriatric assessment & polypharmacy review

We perform comprehensive geriatric assessments covering memory, mobility, mood, nutrition, falls risk, and activities of daily living. If you (or a parent) are taking five or more medications, we offer a polypharmacy review to identify drug interactions, simplify your regimen, and reduce side-effects.

Mental health & dementia support

Depression, anxiety, and grief are common in later life — and very treatable. We screen for depression and cognitive decline, prescribe and adjust medications carefully for older adults, and connect families with community supports including memory clinics, home care, and caregiver respite.

Coordination with home care and family

We work closely with Toronto home-care services, pharmacists, specialists, and family caregivers to make sure you have the support you need at home. Where appropriate, we coordinate visits and communicate with family members (with your permission) so everyone is on the same page.
